A score for a specific pair

ABO compatibility, HLA-B/DR mismatches, biological relationship, sex combination and weight ratio can change the score for the same donor with different recipients.

Comparable risk, not a percentage

An LKDPI of 20 represents modeled risk comparable to deceased-donor KDPI 20 on the study’s 2013 reference scale. It does not mean a 20% chance of graft loss.

Support for clinical review

The model can help compare living donor options, including paired exchange. It does not replace antibody testing, crossmatch, donor evaluation or the transplant team’s judgment.

Living donor model — clinical context required. This tool reproduces the published 2016 LKDPI equation, derived from US transplants performed in 2005–2013. Its ability to predict an individual outcome is limited. It is not the updated OPTN deceased-donor KDPI model, does not use annual KDPI reference tables, and cannot guarantee transplant success or assess donation safety.
